Must have/meet:

• Resident of service area
• Be member of school UNITE club, has attended Camp UNITE or been impacted by substance abuse
• Have Grade Point Average of 2.75 unweighted or 3.75 weighted

Please include in application:

• Typed essay of 500 words or less detailing how they exemplify "I Am UNITE"
• 2 letters of recommendation
• Unofficial copy of high school transcript
